Brazilian prosecutors allege the mining company responsible for the country's largest environmental disaster predicted the potential impact six months previously in a worst case risk assessment.

On the 5th of November 2015, the Fundao tailings dam failed, unleashing about 40 million litres of water and sediments from iron ore extraction in a wave which killed 19 people, decimating wildlife and polluting water supplies.

More than two years later, no one has yet taken responsibility.
