Jennifer Riordan was the first victim of a US commercial aviation accident since 2009, after a Southwest Airlines’ plane engine exploded over Pennsylvania this Tuesday.
Investigators said the broken fan blade was suffering from signs of metal fatigue, the same issue blamed for engine failure on a Southwest flight in late 2016.
The company opposed a recommendation to inspect its’ fan blades after the initial incident, clashing with the Federal Aviation Administration over the decision.
